First Visit With Our Office
WHAT TO BRING WITH YOU ON YOUR FIRST VISIT
You will need to bring your insurance card and a photo ID. If you have any previous medical records please bring them. Dr. O'Heron will need a list of all your medications including the dose and the times you take them. Alternatively, you may want to just bring them with you. Make a list of all your concerns so that you may discuss them with Dr. O'Heron.
YOUR REGISTRATION PROCESS
Please plan to arrive at least 15 minutes early to complete your registration. You may want to down load our registration forms, complete them and bring them with you. They are also available in our office. You will be asked to sign our HIPAA form, as required by law, so that you are aware of our privacy policies.Because of the importance of accurate information for billing and lab services, please make sure we have your updated insurance and contact information. Your most current address needs to be provided at each visit.
PAYMENT
Be prepared to pay your deductibles, co-pays, co-insurance and any other amounts due at the time of your visit. Please remember that your appointment time is valuable. If you are unable to make your appointment please let us know at least 24 hours ahead of time so that we may schedule someone else that needs care.
If you do not, you will be charged a "No Show" fee. You will be called the day before to confirm your appointment. Lastly, please remember if your provider is running late, she may be handling an emergency or complex patient problem. During your visit your provider will give you the same dedicated attention.
